The Intake Whānau Ora Navigator will work with people seeking referral into the agency and identify their needs, complete assessments and on-refer.
Whānau Ora is a whānau-centered service that supports families to achieve their aspirations. Whānau ora recognizes the strengths and abilities that exist within whānau and aims to support and develop opportunities that fulfill potential. Navigators assist with social needs, problem solving, and connection and linkage to other agencies. The navigators will also work with individuals and families on a pathway plan to support goals and future change.
Our Intake Whānau Ora Navigator works with people presenting at reception as a first point of contact for on-referral into our service. The Intake worker also works to maintain clients on waiting lists with any social supports/needs.
Our Whānau Ora Navigator works between clinical service practice to support whānau social needs. The navigator also supports internal and external referrals i.e. Corrections Youth Programme.
