Referral Process

Referrals should come via Community Support Services Access Pathway (CAP).

However, referrals can also be made by self/individuals/whānau, Canterbury DHB, general practitioners (GP), health professionals, government organisations or non-government organisations.

Description

Purapura Whetu provide a whānau centred approach, supporting family members to achieve independence. We understand that each whānau is unique and needs can vary, so we tailor the support with you.

Our mobile team provide regular support to people in the Canterbury area

We support whānau with:

  • Kaupapa Māori care, treatment and wellness plans
  • Practical daily living
  • Identifying and achieving goals
  • Access to social, legal, Māori/Iwi and community services
  • Pursuing a healthy lifestyle
  • Finding training and job opportunities
  • Relationship building with friends and family
  • Advocacy
