Referral Process

To make a referral, please contact the Te Awamutu ICAMHS team on 0800 154 973 or (07) 871 3671.

Referrals can be made by a variety of people including a GP, school, family or a young person themself.

Referrals are triaged by ICAMHS and then referred to Te Whaanau Pataka, Southern Cluster Social Worker for case management.

Description

Te Whānau Pataka is a Cluster member of the Southern Waikato Cluster (Infant, Child and Youth Mental Health & Addictions Services). Our service is here to help infants, children and young people in the Southern Waikato who have moderate to severe mental health and/or addiction issues.

The Southern Waikato Cluster provide specialist services, assessment and treatment programmes, support and advocacy and Kaupapa Māori services to infants, children, young people and their families who are experiencing significant impacts in their life due to mental health and/or alcohol or drug addiction.

Te Whānau Pataka provides case-management and family/whānau support.

Our case-manager works alongside the child / youth and their family, in collaboration with specialist Cluster services, from initial consent/assessment/treatment plan through to discharge. 

Our family/whānau worker works alongside families providing support, education and advocacy, as required, to support family wellbeing within the clinical treatment plan.
