Referral Process

Access to this service occurs via referral from Canterbury DHB Specialist Mental Health Services or general practice.

Referrals can also come via Community Support Services Access Pathway (CAP).

Description

Community Support Workers visit people in their own homes on a weekly basis. This support is utilised to assist people in maintaining wellbeing, and achieving their goals.
