Priority populations are groups that have experienced adverse impacts or are considered at higher risk of the effects from COVID-19 outbreaks in New Zealand. They include Māori and Pacific communities and those living with disabilities.

Description
Te Waka Whaiora Trust is a charitable trust located in Porirua providing a sub-acute whare, service navigation and home based support for mental health.
We were formed in April 2013 as a result of an amalgamation of two key Kaupapa Māori Health Providers in the Wellington region. The two organisations, Te Roopu Whakapakari Ora Trust and Te Roopu Pookai Taaniwhaniwha, undertook a process to merge as a combining resources strategy. Combining our resources, knowledge, workforce, infrastructures and systems not only increased efficiency but it also increased capacity and capability.
Our approach recognises that our transformation to Te Waka Whaiora is not only about collaboration as service providers, but is also represented in our capacity to reconfigure existing delivery models to a better aligned Kaupapa Māori approach.

Referral Process
Access to this service is via referral from the Crisis Mental Health Team and Te Whare O Matairangi based at the Wellington Hospital and Te Whare Marie.
Tangata whaiora come with a plan and are reviewed periodically by their referral team. Tangata whaiora stay could be from one day to twelve months and this is dependent upon the availability of other services in the community

This is accomplished with kaimahi working individually with tangata whaiora and their whānau to access the services and supports that will lessen their dependency on mental health and addiction services. Together a personalised plan is developed and the plan is to achieve these identified goals. Kaimahi within this team also have the ability to refer into Te Whare Tautoko.

Access to this service is via referral from Te Waka Whaiora's Nga Kaihoe Whakatere (Navigation) team or Te Ara Pai Hub and is supported by tangata whaiora case managers.
