Description
THINK Hauora is a not-for-profit charitable trust, that provides primary healthcare services to the MidCentral DHB population. Primary healthcare services are managed and delivered in the community through the PHO.
We provide free services which aim to improve access to health and social services for those people who are experiencing mild or moderate mental health issues.
What we do:
- Assist you/whanau to identify any issues that may impact on your hinengaro - wellbeing
- Provide information about services currently provided within your local area
- Support you to access services relevant to your need
- Work with you/whanau to plan an individualised flexible package of care to meet your identified need
