Description
The Pāhake Whaiora Primary Care service works alongside pāhake (older) people who are experiencing early stage dementia, and age-related conditions including mental health and addiction problems. This community-based service is available for whānau throughout Taranaki. The service can work with community providers to: Navigate pathways for older persons in community and hospital settings, support pāhake and their whānau in this stage of their lives, provide whānau ora based interventions that best help their individual situation such as providing cultural support.
