Description
Mind+ provides Te Whatu Ora funded, individualised, community based support. We work alongside individuals, their whānau, Community Mental Health and Addiction team and anyone else an individual considers to be important in their journey.
The aim of this work is to empower individuals to be in charge of their own health and wellbeing with support to identify and plan for their dreams and aspirations.
The level of support varies according to individual needs over time e.g. from 1hr per month to 2hrs per day. This support is provided to people living anywhere in the wider Te Puke area.
