Referrals are through a WBOPPHO general practice.

Description

The IPMHA model includes an evidenced based approach to providing behavioural health support within general practice.

Through dedicated Health Improvement Practitioners (HIPs) and Health Coaches, the service enables people with mild to moderate needs to access free and immediate mental wellbeing advice and support, where and when they need it. The HIP and Health Coach work as a part of the general practice team to provide brief intervention focused on strengthening self-management strategies.
