Community Dietitians

   

Community Dietitians are key members of the Community Health Team, a community-based healthcare service covering the large Counties Manukau Region. An important focus of the HHC Team is to provide education to empower people to lead healthier lives and support them to continue to live in their own homes.

Locations
There are community-based sites located at Howick (Botany), Māngere, Papakura and Pukekohe. The service is available to people in their own home or at a clinic facility, and care is provided based on an assessment of need.

Service Provided
The Community Dietitian works closely with the client and their family / whānau in their own home (in the case of housebound individuals) or at outpatient clinic settings. The nutrition care process includes nutrition assessment, goal setting, planning a nutrition care plan and ongoing nutrition monitoring. 

Referrals
Referrals are accepted from General Practitioners (GPs) and other health professionals, provided that the individual has consented to the referral
