Description
The Ostomy service provides specialist nursing care which includes assessment, support and supplies to people who have a stoma.
A stoma is a surgically created opening in the body e.g. the abdomen that requires the use of a colostomy, ileostomy or urostomy bag.
What to expect
We will conduct a full assessment and then work with you and your whānau or support person where included to develop a plan to ensure the best outcomes supporting you to manage your condition in daily living.
Appointments are generally in one of our clinic locations, Greenlane or Pt Chevalier. In some circumstances where you are unable to attend a clinic we will arrange an appointment for you at your home.
Referral Expectations
The clinical team who completed your ostomate should make the appropriate referral to our service for your ongoing care. This will be done via an electronic platform called e-referrals and will be considered by our triage team.
