Referral Process

Referrals for Dual Diagnosis Mental Health and Intellectual Disability Community Services can be received from your GP and other health professionals via fax or post to the location nearest you. We do accept referrals from any source, including self-referral, but we prefer that your GP is informed so that both services are involved with your treatment.

Description

The Southern DHB Adult Community Mental Health Teams also provide a range of specialist mental health services for people aged 18 years and over who have been diagnosed with a significant mental illness and intellectual disability.

Appointments will be at the Community Mental Health office nearest you and/or when appropriate, in your home or workplace. We also see people at Medical Centres if appropriate.
