Self referral, GP, DHB clinical services
Referrals for Community Mental Health Services can be received from your GP and other health professionals via fax or post to the location nearest you. We do accept referrals from any source, including self-referral, but we prefer that your GP is informed so that both services are involved with your treatment.

The Community Mental Health Teams are made up of health professionals who provide a range of specialist mental health services for people aged 18 years and over who have been diagnosed with a significant mental illness. We will also see people aged 16 or 17 if they have left school.
Appointments will be at the Community Mental Health office nearest you and/or when appropriate, in your home or workplace. We also see people at Medical Centres if appropriate.
