Counties Manukau Health Memory Team
Public Service, Older People's Health
What is the Memory Team?
The Memory Team was established mid-2013 to improve services for people with memory problems, cognitive impairment and/or dementia within the Counties Manukau area.
We offer specialist, comprehensive assessments for people with cognitive impairment or dementia in their own homes. This gives clients and their whaanau/family a clearer understanding as to what is happening, why and what the implications are for the future.
With knowledge, understanding and support, clients and their whaanau/family can begin to plan for the future and live well with memory loss.
The Memory Team is a multidisciplinary specialist service made up of staff with both physical and mental health experience including:
- Occupational Therapy
- Social Work
- Old Age Psychiatry
The Memory Team works in collaboration with Dementia Auckland.
Your GP will refer you to this service if they think you would benefit from seeing a team of people to assist with any difficulties related to memory problems, cognitive impairment and/or dementia.
