What Health Services Are Provided For Older People?

  • Needs Assessment Service (NASC)
  • Psychiatry of Older People (POP) 
  • Geriatrician assessments
  • Access to publicly funded home, community and residential care for older people
  • Community Gerontology Specialist Nursing Team


Note: Please note below that some people are not available at all locations.

Referral Expectations

All referrals for a Geriatrician assessment or the POPs service must come from your General Practitioner (GP).

Anyone can ask for a Needs Assessment to look at how they can be more able to manage in the community. However, if they have never been seen by the Health of Older People service or if there has been a significant change in their capabilities, a new medical review by a GP, Practice Nurse or specialist will be required.

Before referral, we recommend reading the criteria for access to funded support services listed below.
