Description

Specialist palliative care for people with serious or life-threatening illness.

We provide practical advice and physical, emotional and spiritual care and support. Our holistic care and support extends to family/whānau and carers.

Our focus is on quality of life, and comfort and dignity at the end of life, and helping those who have lost loved ones.

Referral Expectations

Referral Guidelines

People can access specialist palliative care services if:

They are assessed & accepted as having a life-limiting condition with a life expectancy of less than 12 months.

Referrals will be accepted from:

General Practitioners, other healthcare providers, hospital clinical teams, residential care facilities etc.

A patient or their family / whānau / carers

Notes:

The patient must consent to referral.

Hospice House is open Monday to Friday between 8.30am and 5pm. Hospice West Auckland is a community Hospice.

Limited on site parking, but plenty of free parking on the street. Drop off bay at the front door.

Hospice West Auckland provides services free to the people of greater West Auckland.

We are partially funded by the Waitemata District Health Board, with the balance of operating revenue required coming from donations, fundraising events, our annual appeal, trusts and grants, sponsorship and The Hospice Shops.
