ProCare works closely with the Franklin community to develop self-management education programmes that meet the needs of the region's diverse population - including Maori, Pacific and Asian.

Generic, diabetes and pain specific programmes are available for people with long-term health conditions.


Put some LIFE back into your LIFE!

Do you, or someone you care for, have an on-going health problem such as diabetes, asthma, respiratory disease, high blood pressure, high cholesterol, being overweight, at risk of diabetes, kidney disease, heart disease, depression, anxiety, arthritis or any other health problem?  

Find out what you can do to keep yourself or your loved ones well by coming along to the My Health Our LIFE programme. Recommended by doctors and led by health professionals and peer leaders, this is one of the most useful and practical courses you can attend.

How do I access this service?


Self referrals are accepted otherwise you can be referred by a health professional e.g. practice nurse, doctor or physiotherapist. 


Mon – Fri 8:30 AM – 5:00 PM

Course times vary.

Group sessions can be organised in the evenings or weekends.

This is a free service and is open to anyone over 18 years old living in Counties Manukau.
