Description

Dr Jeremy Steinberg is dual trained in Musculoskeletal Medicine and General Practice. He splits his time between Musculoskeletal Medicine at Marua Medical Specialist Health Centre, and General Practice at Ellerslie Medical Centre.

At Marua Medical Specialist Health Centre Jeremy sees patients aged 13 and older referred for primary chronic musculoskeletal pain problems (he doesn't see children 12 and under). He has particular interests in:

  • Chronic musculoskeletal problems that have been difficult to diagnose
  • Chronic spinal pain (cervical, thoracic, lumbar, sacroiliac, and coccyx)
  • Radicular pain and radiculopathy
  • The intersection between musculoskeletal medicine and neurology (certain but not all headache, neuromuscular, and neuropathic pain disorders)
  • Complications from joint hypermobility

He is also trained to do image guided injections under ultrasound and fluoroscopy for various conditions (particularly spinal) plus radiofrequency neurotomy for facet joint pain. He completed a fellowship in Spinal Interventional Pain Management in Auckland in 2022-2023. He works closely with other local Musculoskeletal Medicine specialists, as well as colleagues in the Allied Health fields.

Please note: For ACC funding there needs to be a clear injury link to a specific event. Most chronic pain is not related to injury. For example, assessments around hypermobility are not covered under ACC. Chronic pain that predates a minor aggravation is generally not covered.

What is Musculoskeletal Medicine?
Musculoskeletal medicine involves the diagnosis and non-surgical treatment of disorders of the musculoskeletal system (bones, muscles, cartilage, intervertebral discs, ligaments, tendons, and other connective tissue). Musculoskeletal disorders may occur as the result of injury or be gradual in onset such as osteoarthritis and tendinopathy.

Management of musculoskeletal disorders includes a wide range of treatments such as: patient education, pain management strategies, manual therapy, exercise and rehabilitation programmes, specialised diagnostic and therapeutic injection techniques, and medication.

There is overlap between Musculoskeletal Medicine, Rheumatology, Sports Medicine, Pain Medicine, and Orthopaedic Surgery. Your GP or allied health provider will be able to guide you on the best type of doctor to see for your problem.

  • Musculoskeletal Medicine Specialists usually see patients with musculoskeletal pain problems that are due to injury or the ageing process. There is an emphasis on diagnostic accuracy where possible.
  • Rheumatology has a focus on joint pain that is due to a problem with the immune system such as Rheumatoid Arthritis but may also see patients with injury and age related conditions.
  • Orthopaedic Surgeons are the only specialists able to provide surgical options for pain and are also experts in fracture management.
  • Sport and Exercise Medicine is the medical care of the exercising person. This may include injuries or any general medical concern as it relates to exercise.
  • Pain Medicine has an emphasis on complex pain where the problem isn't directly curable (for example pain in multiple different areas of the body) with the aim of improving function and quality of life. Most Pain Medicine physicians work in the hospital chronic pain clinics where there is a strong emphasis on using a team based approach to care.

Referral Expectations

Questionnaire: Your GP or allied health provider will have referred you for the consultation. A three page form will be sent to you to fill out which has questions about your health and pain problem. Please send it to admin@maruamed.co.nz so that an appointment time can be given. The questionnaire is very important (especially the pain diagram) because it allows us to have a better baseline understanding before seeing you and it means that it is less likely we miss something significant.

ACC Referrals: If you are being referred under ACC, please provide us with the original injury claim. You are welcome (but not required) to provide any subsequent claims that may have aggravated the original injury. The referral also needs to contain details about the mechanism of injury.

For ACC the problem has to be directly related to the injury claim. For example if you have a neck injury claim we can only see you for your neck, we cannot for example see you for your low back.

Previous Records: If you have previously been seen by another private specialist please send the clinic letter to us beforehand. If it was a public specialist in Auckland or Northland then we can access the letters online with your permission. We can't access public letters outside of these two areas (e.g. we can't access Waikato records).

Second Opinion Policy: Jeremy does not routinely accept referrals for second opinions if you have already seen another Musculoskeletal Specialist for the same problem. There is only 1 musculoskeletal specialist per 200,000 population (compared to 1 in 15,000 for orthopaedic surgeons), and so there is massive unmet demand for first opinions. This policy doesn't apply if you have seen a different type of specialist.

Medicolegal Matters: Jeremy is not accepting referrals for medicolegal opinions and reports at this time, such as support for challenging ACC cover decisions.

Fees and Charges Description

General: Please check that you are covered for the problem being referred, whether that is ACC or private insurance. The most common problems we encounter are: people forgetting that their private insurance policy only covers surgery and not consultations or scans, forgetting that they only have partial cover and need to pay a surcharge, and forgetting that they have an exclusion for their problem in their policy due it being pre-existing.

ACC
: The service is ACC registered and accredited. Approved for the following regions: Rodney District, North Shore City, Auckland City, Waitakere City, Manukau City, Papakura District, Franklin District. Please note that the pain problem needs to be relevant to the ACC claim for funded treatment (e.g. ACC do not fund treatment for osteoarthritis).

Southern Cross: Jeremy is a Southern Cross Affiliated Provider for appointments and most procedures. This means we can process your prior approval and invoice for you if you have your membership number and referral letter.

If your problem is very complex (e.g. a long history of unresolved widespread pain) there is limited availability of longer 90 minute initial appointments. The referrals are triaged to determine if this is needed.

NIB: We can submit a prior approval request on your behalf for consultations. Please advise us well in advance if you want us to do this for NIB (2 week notice). NIB require applications for any other service such as injections to be submitted by you rather than us (we can provide a quote). Please note that most NIB policies do not cover injections for pain (even steroid/cortisone).

Other insurance: If you have medical insurance with another insurance company we recommend that you contact them to arrange prior approval. Otherwise, and if you are not insured, your account will be required to be paid in full at the time of service.

Platelet rich plasma injections are excluded from ACC and Southern Cross funding.

Please contact the clinic for details of our consultation and procedure charges if required.
