What is the Community Geriatric Service?
The Community Geriatric Service (CGS) is a community service for GPs and nurses working with people who are in CMDHB Aged Related Residential Care Services. The Specialist Geriatrician and Clinical Nurse Specialists provide support to the Residential Care Health Teams in the area of Gerontology. The aim of this service is to improve the health and outcomes for older people in residential care within the CMDHB region.
The Community Geriatric Services Team includes:
Clinical Nurse Specialists - the nurses in this team are specially trained in conditions affecting older people and may be involved in your follow up to provide practical advice at home and provide feedback to your specialist or GP. The Clinical Nurse specialists will be able to link you to other health professionals e.g. Needs Assessors who are experts in arranging support for you. They refer you to NASC for assessment to help you in the home or going to a residential care facility.
