Community Health Service



Sport Auckland Green Prescription (GRx) is a referral based health and wellness programme for adults 18 years and over. It is best suited for adults who would like support and motivation to get active and improve their lifestyle. It is delivered through group sessions in various communities in Auckland and Counties Manukau. Sport Auckland also delivers Green Prescription 'Active Families' to children aged 5 - 17 years of age. Please see this link for information relating to our Active Families programmes.

We are pleased to advise you that Primary Care based referrers can now refer patients via e-referral using CareSelect (HealthLink forms). Using this referral pathway is quick, easy and secure. Simply launch HealthLink from your electronic medical record (EMR) system to use HealthLink’s CareSelect eReferral service.

  • Search for us using our organisation name, EDI (sportakl) or any key word relating to our service and compose the referral. CareSelect makes it easy for you to provide relevant patient information in your referral to us.


What can you expect?

  • Initial one-on-one consultation with a Healthy Lifestyle Advisor
  • Weekly group sessions involving physical activity and/or lifestyle education
  • Group workshops by a Registered Dietitian
  • Group workshops by various health agencies
  • A healthier and more active lifestyle!

Translation services are available in various languages across both DHBs.
