Description

Sport Auckland Green Prescription (GRx) is a referral based health and wellness programme for adults 18 years and over. It is best suited for adults who would like support and motivation to get active and improve their lifestyle. It is delivered through group sessions in various communities in Auckland and Counties Manukau.

Sport Auckland also delivers Green Prescription 'Active Families' to children aged 5 - 17 years of age in ADHB. Please see this link for information relating to our Active Families programmes.

We are pleased to advise you that Primary Care based referrers can send eReferrals through CareConnect. Using this referral pathway is quick, easy and secure. 

  • CareConnect is the prefered referral method for all Primary Care 

What can you expect?

  • Initial one-on-one consultation with a Healthy Lifestyle Advisor
  • Weekly group sessions involving physical activity and/or lifestyle education
  • Group workshops by a Registered Dietitian
  • Group workshops by various health agencies
  • A healthier and more active lifestyle!

Translation services are available in various languages across both DHBs.

During the period 1st October - 21st December 2019 the Green Prescription service will only be delivered in Otara and Manurewa. All clients referred to the GRx service living outside of these communities will be contacted, put on hold and notified of the new programme starting in January 2020 which they can engage with.

E-referrals can be made by Primary Care Health Professionals through Care Connect 

Step 1 - Go to ‘Green Prescription’ in the CareConnect eReferral platform
Step 2 - Please provide a clear reason for the referral and give the patient a Green Prescription flyer
You can advise the patient they will be contacted within 4 working days
