The Community Based Rehabilitation Team (CBRT) provides a client centred, culturally appropriate, goal focused rehabilitation service in the environment of choice, across the spectrum of physical function, activity and social participation. Currently the service is limited to stroke survivors, and it is hoped that the service will expand to other diagnoses in the future. Of the CMDHB catchment we currently do not cover the Franklin/Pukekohe district. This area is serviced by Home Health Care.
The team includes a Co-ordinator, Social Worker, Physiotherapist, Speech Language Therapist, Dietitian, Occupational Therapist, Specialist Doctor and Rehabilitation Assistant with administration support.
Community Based Rehabilitation takes place in the environment of choice, this includes home based rehabilitation, community gymnasiums and local swimming pools, and other areas where people would like to access such as the beach and shopping malls.

Hours of Service:
Monday to Friday 8.30am to 4.30pm
On receipt of a referral or discharge from hospital, the client should be contacted within 5 working days. Upon acceptance of the referral the client should be visited within 10 working days, in their own place of residence/ home, or in an appropriate setting.
